ESRD Facility Survey File

The provider characteristics and aggregate patient population data portion of PMMIS captures certification and other types of information on providers approved by Medicare to furnish dialysis and transplant services, and contains aggregate patient population counts. The End Stage Renal Disease (ESRD) Facility Survey File data are collected annually by HCFA from all facilities certified to provide Medicare covered renal dialysis and transplantation. The survey, which includes the entire United States, uses HCFA Form 2744 and it encompasses the full calendar year. Geographical data are included to the level of zip code for the facility. Each record contains information about the facility itself and information about the number of patients served, the number of dialysis treatments provided, and the number of kidney transplants performed. The aggregate population data are the only population counts that include services to both Medicare and non-Medicare patients.
